Move a resource from R4 to R5

Paste a FHIR R4 resource as JSON. The helper compares the elements you actually use with the official "Changes from R4" tables in the FHIR R5 (5.0.0) specification and shows what affects you.

Steps

1. Open R4 to R5 Helper.
2. Paste a resource, or press Load example.
3. Press Show R5 changes (or ++ctrl+enter++).

What you get

• Each change that touches an element in your resource, in plain English, with what to do
• Elements that are newly required in R5. An element is listed only when your resource has to include it: every element above it is required too, or its parent is already in your R4 resource. Required parts of new optional elements are counted with the other changes below, not listed as things to add.
• A count of other changes to the resource that do not touch your data
• A link to the official R5 page for the resource
• A draft with simple, mechanical edits applied, for example a renamed element, a single value wrapped in an array because it can now repeat, or reasonCode moved into reason as a CodeableReference. Anything that needs a decision is listed, not changed.

The change data comes from the "Changes from R4" tables in the FHIR R5 specification (hl7.org/fhir/R5, CC0), collected on 2026-10-25 for 146 resources.
